No Show Policy

We set time aside for same day sick visits and appointments for acute problems. Missed and cancelled appointments impact the care of other patients. There is a $25 fee for missed appointments.   There is a 3 No Show appointment rule. After three no-show appointments you will be discharged as patient effective on the date of the last missed appointments.


* We will continue to fill essential medications for up to 60 days.

Rescheduling and Cancellation Policy

Every morning and afternoon our providers review their patient's labs, recent imaging and upcoming visit concerns. We ask that patients call in at least 4 hours prior to their appointment to reschedule.  Appointments that are not rescheduled/canceled at least 4 hours in advance may be documented as a no-show visit (please refer to the No Show Policy).

Active Insurance Policy

It is the responsibility of the patient to update the office of changes in health insurance coverage and plans prior to appointments. It is also the responsibility of the patient to maintain active health insurance or pay cash for their visit. Failure to update the office staff may result in the patient being directly billed for their visit.

Late Policy

We value the time of each patient and pride ourself in allowing our patients the time to share their concerns. Arriving late to an appointment shortens not only your visi time but possibly the next patient as well. For this reason, if patients  are 15 minutes late for a scheduled appointment will be rescheduled to a later time or another day.

Payment Policy

Payment for services are due at the time of your visit. If you have a deductible plan, you will be expected to pay at least 50% of the estimated cost of your visit. 

Reception

Change of Provider Notification

It is the responsibility of the patient or caretaker to notify their personal health insurance company of changes in their primary care provider. Patients who forego this process may be subject to higher co-pays. 

Credit Card

Outside Facility/Cost

All diagnostic testing (laboratory "blood/urine/stool" test, imaging, etc) are not processed in office. These costs for these tests are controlled by the facility in which order requisitions are sent to.
