The Patient Services Department at Jackson's Health improves access to health care and assures that patients can access health care without insurance, and regardless of insurance status. Our BIPOC owned sites offer access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program, with discounts available based on your current household size and income only. The program can cover you for up to 12 months—a new application is required yearly or when your household size and income changes, whichever is sooner.

Sliding Fee Scale Discount Program
The Sliding Fee Discount Program assures that patients without health insurance coverage have access to all primary care services. Eligibility for the sliding fee scale discount is based upon poverty guidelines established by the federal government and are determined by family size and income.

Privately Insured Patients
Patients with high-deductible health plans that require out-of-pocket expenses in excess of $2,500 annually will be extended the opportunity to apply for a sliding fee scale discount. Determination is based on documentation and verification requirements. If eligible, We will apply the sliding fee discount by charging a fee no greater than what would be charged to a patient that has no public or private insurance in a defined income category.

To apply for sliding fee services and alternate payment arrangements you will need to provide proof of household income. We will require a current valid photo I.D. and any one of the following items:

All pay stubs from the past 30 days

Letter from your employer or a declaration of income

Unemployment benefit letter

What is Direct Primary care?
We are keeping “Your care, Your way” by keeping our focus on you and your well-being, rather than the ever-increasing administrative tasks that detract from the time we can spend with you.
We believe this will allow us to care for you and your family in the way that you want. In place of billing insurance, Direct Primary Care will involve an affordable monthly membership fee that covers the services listed below. By eliminating the middleman, we can keep costs low and use our time to do what we are trained to do, help you maintain your health.

Membership benefits:
Easier access: generally same day or next day appointments will be available.
Cost efficient: Only one monthly payment for covered services.
Is Direct Primary Care Insurance?
No, we recommend that you keep your current insurance plan. If you can switch to a PPO plan, this often works better with Direct Primary Care.
