Jackson's Health: Family Health NP PLLC, is a private office of primary care nurse practitioners located in Brighton, NY. We provide equitable and quality care to the Rochester community. We are a privately owned group that cares for patient's that are 17 years old and up. Our providers understand the needs of the Rochester community and are dedicated to optimize health and wellness. Our providers are Board Certified in Family Health and are dedicated to providing high quality, compassionate and equitable care for our patients and their families. Judgement free care.
Missed Appointments
We set time aside for same day sick visits and appointments for acute problems. Missed and cancelled appointments impact the care of other patients. If you have to cancel or reschedule your visit, please do so 24-48 hours in advance. There is a $25 fee for missed appointments. There is also a 3 No Show appointment policy. After three no-show appointments you will be discharged as patient effective on the data of the last missed appointments. *
* We will continue to fill essential medications for up to 60 days.
Insurance Policy
We are now accepting NYSHIP Empire Plan, Aetna, MVP, MVP option, CIGNA, United Healthcare, Tricare, Emblem Health, Blue Cross Blue Shield, Blue Choice, Excellus, Fidelis, Anthem, Medicaid and Medicare!
We do not accept Independent Care, Molina or Monroe Plan.
If you do not see your insurance plan, please notify our team and we can possibly add your insurance carrier!
If you are a current patient, please use the patient portal to contact your provider. This will allow us to review your connected medical chart and provide personalized care.
